Please be informed that the Department of Education - Central Office does not keep learners’ school records. You may refer to the Guidelines on Learner's School Records in the DepEd Order No. 54, s. 2016 titled "Guidelines on the Request and Transfer of Learner’s School Records" regarding your request for a Form-137 (Permanent Record), Form-138 (Report Card), and other school records for your information and guidance:

For further clarification, you may directly seek the assistance of the school where you graduated or the Schools Division Office near your area.
