Requests for a Vaccine Card

Requested from SRWD2 by J. APELO at 11:51 AM on May 06, 2022.
Purpose: To request for a new vaccine card.
Date of Coverage: 04/06/2022 - 04/06/2022
Tracking no: #SRWD2-341804093682

APELO 11:51 AM, May 06, 2022

Good day! I would like to raise a concern regarding my vaccination record. I lost my vaccination card yesterday at the MOA Arena. I'm fully vaccinated but when I checked my record through, it says that I have a record.

I was vaccinated in Sta. Rosa Laguna in Enchanted Kingdom. My first dose was on November 10, 2021 and the second dose was at December 02, 2021. As I stated above, I lost my vaccination card and I would like to know the steps to have new vaccine card if it's possible. Thank you very much!
