Hi, I would like to request for
(1) a copy of the Memorandum of the then Secretary Jose L. Atienza dated November 27, 2009 with regards to the requirements of securing an Tree Cutting and/or Earth-Balling Permit; and any superseding Memoranda thereafter, if ever there were, and
(2) a sample copy of the Approved City Endorsement Plan as stipulated in the said Memorandum. Thank you.

Attached is a copy of the Memo dated November 27, 2009 by DENR Secretary Jose Atienza for your ready reference.
Please be informed that your second request, the City Endorsement Plan, is not anywhere written in the aforesaid Memorandum. If you are mistaken with the "endorsement of LGU concerned" from the City Endorsement Plan you are mentioning, kindly be advised that most LGUs are acquainted with this kind of tree cutting endorsement letter and you may request with them directly.
