Hi there!

I'd like to request an Earthquake Hazard Assessment of Lots 5446, 5447, and 5448 of Psc-52, Taal Cadastre, Taal, Batangas.

The purpose of the document is to verify the fault line location in relation to the lots and provide an assessment of any hazards.

I'm happy to provide any files needed for this.

While our aim is to provide information whenever possible, in this instance this Office in this instance we would need for you to resubmit your request as this is considered part of our frontline services, also indicated in our Citizen's Charter. Please send an email to ggrdd.mail@phivolcs.dost.gov.ph. , addressed to Renato U. Solidum, Jr., Officer-in-Charge of PHIVOLCS with all the details for the hazard assessment: location/vicinity map, satellite image, Transfer Certificate Title.
Your right to request a review
