The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center. If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
