You asked for Required Documents for Medical Assistance.

Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not provide financial/medical assistance. However, you may wish to contact other government offices such as DSWD with regard to your request. In addition, please be informed that PhilHealth provides benefits for hospitalization. An active PhilHealth member can avail of the PhilHealth benefit prior to discharge to the hospital via case rate packages.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to <Emmanuel R. Ledesma, Jr. - Acting President and Chief Executive Officer (APCEO), nrsantiago@philhealth.gov.ph, or privacy.dpo@philhealth.gov.ph>.
