I am Jaimie An Balisi, I have inquiries regarding my lost Philhealth ID:

1.  What are the requirements I need to present to the Philhealth office to get another ID?

2. If ever there are IDs needed, I have no other valid IDs aside from my birth certificate is that enough?

You asked for LOST PHILHEALTH ID.

Response to Request

The procedure for PhilHealth ID replacement is the same as that for new regular ID applications.  Kindly bring the accomplished PMRF Form and two (2) valid IDs for verification.

Your right to request a review
