May I formally request from your office a certification of date of birth of my mother with following details:

Complete Name: Carmelita Estaño Dolorican
Date of Birth: November 19, 1951
Place of Birth: Bohol

The reason for this request is that she is trying to comply with the notice of deficiency issued by GSIS in relation to her retirement claims.
