Good day. I'm John Carlo Cabalsa, requesting to block my samsung galaxy A10s phone using imei no.351579116272426 and sim card no.09477249048 and 09076531487 to prevent using all the access to my accounts active to that unit and sim. Kindly assists me. Thank you.

Thank you for your request dated January 13, 2020 04:37 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request:

You asked for STOLEN/MISPLACE SAMSUNG PHONE UNIT.

Response to Request:

In reference to the above request , may we refer you to visit/contact our Consumer Welfare and Protection Division (CWPD) for the Blocking of Lost/Stolen Mobile Phone/Unit. Our office is available from Monday-Friday 8:00AM - 5:00 PM. Or you may send your formal written complaint to the CWPD thru: consumer@ntc.gov.ph or call at 8-9213251 & 8-9267722.

The following are the requirements:

1.  Duly accomplished Request for Blocking Form

2. Proof of Ownership, ANY of the following:
    2.1 Copy of Official Receipt of the mobile phone
    2.2 Box of the mobile phone with International Mobile Equipment 
           Identity (IMEI)
    2.3 Copy of duly notarized Affidavit of Loss
          Note: The Affidavit of Lost must indicate the IMEI of the mobile 
                      phone and/or the SIM to be blocked.

3. Copy of valid Identification
    3.1 Any government-issued ID or Passport
    3.2 For students, school ID
    3.3 For cases when ID is not available, Birth Certificate OR NBI 
          Clearance

Supporting Documents for Representative(s)
1. Authorization letter duly signed by the applicant and valid ID of the authorized representative.
