Dear Ms. Angelica Cara Madamba:

I refer to my letter of Apr 29, 2022 11:29:06 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return.

In order for us to work on your request, we are seeking for further clarification/s specifically on the following details:

1. Are you a Local Employee for the past three (3) year/s 2019-2021? 
2. Or a Self Employed Individual (With Business, Practicing One's Profession or Under Job Order)?
3. What ITR form/s are you referring to? - Is It BIR Form 2316/1700 for Employees or BIR Forms 1701/1701A for Self-Employed Individuals?
4. Name of your Employer/s for the year/s 2019 and 2020?
5. Employer's Business Address; and 
6. Employer's Business TIN (if applicable).

Kindly email the needed information at sherry.ann.arcita@bir.gov.ph (indicate the Tracking No for reference #BIR-495986294275) as this contains sensitive information and for us to properly endorse your request to the concerned Revenue District Office (RDO).

Lastly, you may directly request your BIR Form 2316 for Calendar Year 2021 from your employer as they are the main source of this document. It is also mandated by law that every employer shall issue BIR Form No. 2316 to every employee before the close of such calendar year, or if employment is terminated, on the day on which the last payment of compensation is made.

However, if you are self-employed individual your ITR could be BIR Form 1701 OR BIR Form 1701A which was supposedly filed last April 18, 2022 and you should have a copy of the said form/s.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Jul 22, 2022 07:26:13 AM, your request will be closed.
