Please furnish me with a copy of the Account Information Form (an attachment to Form 1701). I tried to download from BIR forms but it is not available at the Account Information Forms page.

Greetings!

Thank you for your request dated Mar 30, 2023 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Response to your request

In reply, upon consultation with the concerned office (Taxpayer Service Programs and Monitoring Division) the Account Information is no longer required, instead a Financial Statement (FS) is required as attachment to your Financial Statement.
