Hi! I would like to request another vaccine card because mine got lost. I dont have any pictures of my vaccine card however I remember when and where I got vaccinated. I got my first dose at August 27, 2021 in Robinsons Antipolo and my second dose at October 12, 2021 at the same place under the name Elcent Den Teoxon Nuñeza. I tried searching for my record in VaxCertPh, it says that I donthave any record. I hope my request gets fulfilled for I will be using it for our weekly church service.
