Hi can i get a copy of my vaccination card? I lost my card and i put somewhere so that i cant go on our school to get my documents and attend face to face class

Response to Request

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
