Hello, I would like to request a replacement vaccination card. I was vaccinated with my first dose on October 22 2021 through ayala malls in bacolod city. The doctor in charge was Dr. CHRIS sorongon. I completed my vaccination on November 12 2021 and recently lost my vaccination card.

with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
