Hi Madam, 

my niece vaxcert card has incorrect middle name, it should be “PELICANO” not Peliciano. Certificate id is 303329185. 
Please help to change it.

Response to Request

Your FOI request is approved. 

Please be informed that the encoding of
vaccination records is done by the concerned
vaccination center. The vaccination record may
have been incorrectly encoded in the Vaccination
Information Management System (VIMS) by the
vaccination center.
Kindly coordinate with the LGU where you
received your vaccination and request for
assistance to correct your vaccination record and
certificate.
