         This is Agatha Jesusa Cedo from Santa Rosa, Laguna. I just want to ask, how to replace my Covid19 Vaccine Card? My vaccine card got lost and I need it for the face to face class. May I know the requirements needed to request anothe or new Vaccine Card?

with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
