Hi! I would like to change my birthdate and name on my vaccine card. There must have been a wrong input about my name and birthdate when i was signing up for my 1st dose. Please kindly change it from John Cedrick Escaba De Guzman to John Cedric Escaba De Guzman, also please change my birthdate from May 05, 1997 to May 04, 1997. I might run into some problems in the future with any of my government documents.

Your FOI request is approved. 

Please be informed that the encoding of
vaccination records is done by the concerned
vaccination center. The vaccination record may
have been incorrectly encoded in the Vaccination
Information Management System (VIMS) by the
vaccination center.
Kindly coordinate with the LGU where you
received your vaccination and request for
assistance to correct your vaccination record and
certificate.
