I would like to raise a concern regarding my vaccination record. I lost my vaccination card this September. I'm fully vaccinated but I don't have other copy of my vaccination card. I was vaccinated in Municipality Covered Court, Poblacion, General Mamerto Natividad, Nueva Ecija. My first dose was on October 25, 2021 and the second dose was at November 15, 2021. As I have said above I lost my vaccination card and wasn't able to take photo of it, but I do have pic of proof.
