Hi!

 I would like to request another vaccine card because my niece's got her lost. We don't have any pictures of her vaccine card however she remember when and where she got vaccinated. She got my first dose at November 13, 2021 in Lucena City, Quezon and her second dose at December 11, 2021 at the same place under the name Shayne Marjoree Moog. We tried searching for her record in VaxCertPh, it says that she don't have any record for my 2nd dose. I hope my request gets fulfilled for we will be using it for our trip in Iloilo next week.

Response to Request

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
