Greetings FOI, I would like to request information regarding my Vaccine Card/Certificate due to that I lost it while I was getting the requirements for my upcoming job. I lost it within the time frame of June 7th to 15th. And I honestly don't know where I lost it. It would be much appreciated if I could get any information regarding my vaccination so that I can enter the establishment where I will be working on to. 

My first dose was 10/01/2021 and my second dose is 10/29/2021
My Vaccine was Sinovac and My name is Kevin Roldan Cruz.

Response to Request

Your FOI request is approved. 

About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
