We were able to determine that this particular information is already available online and therefore cannot be counted as a valid FOI request. You may retrieve your requested information from the DOH National Health Facility Registry (NHFR) which is the masterlist of all health facilities including the head of the facility. You may visit the NHFR website at https://nhfr.doh.gov.ph/rfacilities2list.php

Your right to request a review
