Greetings! I would like to raise a concern regarding my vaccination card. I lost my vaccination card last May. May I know if I can request for a new vaccination card and what process must I do to obtain it? 

Below is my information

I was vaccinated in General Trias, Cavite. My first dose was on August 10, 2021 and second dose was September 1, 2021.

Response to Request

Your FOI request is approved. Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.

Kindly coordinate also with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. Thank you.
