Hi, I would like to request my  vaccination certificate I do  vaccinated last my first dose September 6 2021 Sept 27,2021 and my 1st Booster it was June 6,2022 in Cainta One Arena 
I already check and try to request in vax cert website but they says my record not found my I know why they not show up my record on their website? Pls I really need my vaccine certificate for my travel I hope someone can help me here or assist me for my vac cert



I will be traveling to Australia on January, 13,2023

Response to Request

Your FOI request is approved. 

Please be informed that the encoding of
vaccination records is done by the concerned
vaccination center. If your vaccination record is
unavailable, the record may not have been
uploaded yet in the Vaccination Information
Management System (VIMS) by the vaccination
center.
For further assistance, you may call the Vax Cert
Customer service Hotline. Here are the contact

Kindly coordinate also with the LGU where you
received your vaccination and request for
assistance to generate your vaccination
certificate. Thank you.
