Hi! I would like to follow up my vax cert and a copy also.

To whom it may concern: 

This is to bring to your attention that I have been trying to access my vaccine certificate numerous times.

However, your system always declines it and always tells me that there is no vaccination record found to my addition shot and it's been 3 months since I took the 3rd dose. Thank you so much.

I am really hoping you can help me. Urgently needed in the Agency thank you

Your FOI request is approved. 

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.

Kindly coordinate also with the LGU where you received your vaccination and request for assistance to update your vaccination certificate. Thank you.

Your right to request a review
