Hi.

I would like to request for the correction of my middle name in my vaccination certificate. Details below:

Wrong middle name - Agraman
Correct middle name - Agramon

For my vax cert for my 1st 2 primary shots and my 1st booster shot, my middle name was spelled correctly. But for the vax cert with the 2nd booster shot, my middle name was spelled incorrectly as AGRAMAN, instead of the correct spelling of AGRAMON. This, although I entered the information correctly.

Response to Request

Your FOI request is approved. 

Please be informed that the encoding of
vaccination records is done by the concerned
vaccination center. The vaccination record may
have been incorrectly encoded in the Vaccination
Information Management System (VIMS) by the
vaccination center.
Kindly coordinate with the LGU where you
received your vaccination and request for
assistance to correct your vaccination record and
certificate.
