Hello!

I would like to request correction of my middle name in my vaccination record and certificate on vaxcertph. 
The website / vaxcert record for my name is set to Claveto as my middle name but it is Clavito
Below are all the details.

Birthday: November 09, 1997
Sex: MALE

You asked for change my misspelled middle name.

Response to Request

Your FOI request is approved.
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center.

Kindly coordinate with the LGU where you received your vaccination and request for assistance to correct your vaccination record and certificate. Thank you.
