Hi, 
I lost my vaccination card and I would like to know how I can get a copy. I wasn't able to take a photo of my vaccination card but I remember when and where I was vaccinated.

Hope to get the copy asap.

Response to Request

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
