I want to ask for financial assistance for my uncle's hospital bills. That cost 105 thousand pesos. He is currently confined at San Pedro Calungsod Medical Center Inc. My family is already suffering from loans and debt, we cannot afford to pay 105 thousand pesos to release my uncle.

In addition, kindly submit the following documents to the Malasakit Program Office or the Malasakit Centers

1. Medical Certificate or medical abstract
2. Certificate of Indigency
3. Drug or Medicine prescription or treatment protocol, as applicable
4. Laboratory or diagnostic request
5. Hospital bill

Your right to request a review
