We are 4th year BS Civil Engineering students from National University - Manila. We are conducting our capstone design project entitled, "Kalinga: A Proposed Design of Outpatient Clinic in Barangay Boboy, Agno, Pangasinan." In connection with the development of our study, we are requesting an opportunity to obtain copies of the following:

1. Leading Causes of Morbidity and Mortality in Agno, Pangasinan from 2015 to 2023.
