Hi! I would like to request another vacvine card because mine got lost. I don't have any pictures of my vacvine card but I do remember when and where I got vaccinated.  I got my first dose at December 1, 2021 at MLQ Court, Caloocan City and my second dose on February 10, 2022 at SM Tungko San Jose Del Monte Bulacan under the name Bienne Marie MAngielette L. De Guzman. I hope my request gets fulfilled for I will be using it on my out of country business immediately.

Response to Request

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
