To whom it may concern: 

This is to bring to your attention that I have been trying to access my vaccine certificate numerous times since last week, June 20 2022

However, your system always tells me that there is no second dose vaccination record found(06/28/2022). I have already submitted a copy of my vaccination card and my id to your system.

I am really hoping you can help me.

Your FOI request is approved.
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.

For further assistance, you may call the Vax Cert Customer service Hotline. Here are the contact details:

Kindly coordinate also with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. Thank you.
