Hi, I would like to request my vaccine record updated with my Booster shot. When I downloaded the certificate it didn't show my booster record on December 29, 2021 (Pfizer). The summary record before downloading the file says you don't have the record on file. I have a vaxcert card to prove it. Hoping for your assistance, please. Thank you!

Your FOI request is approved. 

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.

Kindly coordinate also with the LGU where you received your vaccination and request for assistance to update your vaccination certificate. Thank you.

Your right to request a review
