Hi!

I would like to request for correction on my misspelled surname in my vaccination record. I tried to search for my name and record in vaxcert.ph but it seems there are no records found. My surname in the vaccination card is spelled as TABIA but it should be TAPIA. I already informed the LGU's about this error on the site but it seems it wasn't changed yet.
Below are the details.

First Name: Alexis
Last Name: Tapia
Middle Name: Mahino
Birthday: July 05, 1996
Sex: Female

1st Dose: June 19, 2021

2nd Dose: July 10, 2021

Vaccination Location: Ayala Centrio Cagayan de Oro City

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center.

Kindly coordinate with the LGU where you received your vaccination and request for assistance to correct your vaccination record and certificate. Thank you.
