Hi. This is to formally request to change the vaccine brand of my 1ST DOSE from Pfizer to SINOVAC on my VaxCert. Upon checking my VaxCert online, I noticed that my 1st dose has been wrongfully inputted as Pfizer even though I was vaccinated with SINOVAC.

I have already submitted a request on the Vaxcert many days ago but there has been no response so far. I am currently waiting for the update confirmation before I am able to access my VaxCert. Kindly refer to the ff.:

Response to Request

Your FOI request is approved. 

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center.

Kindly coordinate with the LGU where you received your vaccination and request for assistance to correct your vaccination record and certificate. Thank you.
