Dear Sir/Ma'm,

I have made a request to vaxcert.doh.gov.ph to validate my vaccination card and credentials and has been pending ever since. I have made numerous attempts to contact DOH by email and phone. I have also created numerous attempts for track request but they always end up Closed with no update in the system. My very latest request attempt was on January 10, 2023. Kindly assists as I need an update/response regarding my request. Thank you and Regards.

You asked for Vaccination Certificate.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
