I would like to request correction of my name in my certificate on my Vaxcert ph record.

The website Vaxcert record of my name is set to "Jonalyn David Pama" my name should be set as "Jhonalyn David" 

Bellow are all the details:

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center.

Kindly coordinate with the LGU where you received your vaccination and request for assistance to correct your vaccination record and certificate. Thank you.
