May 24, 2022 Dear Ma. Anjanette Lacaya, Greetings! Thank you for your request dated May 23, 2022 01:12:56 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for Information of COVID 19 Vaccination Certificate. Response to your request While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. Since your brother were not vaccinated in the Philippines for any of your doses, Unfortunately, the vaccination records may not be accessible by the central database which holds the data of recipients of the COVID-19 vaccine from the Philippines. Kindly request for proof of vaccination (i.e. vaccination card/vaccination certificate) from the country where your brother received his primary dose series, provided it has all the necessary information (personal details and vaccination details) Thank you For further assistance you may review the FAQ section of the eFOI site: https://www.foi.gov.ph/help.
