Information of COVID 19 Vaccination Certificate

Requested from DOH by M. Lacaya at 01:12 PM on May 23, 2022.
Purpose: To get booster shot vaccine against COVID 19
Date of Coverage: 05/23/2022 - 05/23/2022
Tracking no: #DOH-026694957911

Lacaya 01:12 PM, May 23, 2022

Good afternoon! I would like to ask information of this inquiry:

How can my brother get his vaccination certificate if he was vaccinated in Iran? We tried to go to the link provided by the vaccination personnel from Iran, unfortunately, we cannot go through the website given. He is trying to get his booster shot against COVID 19 in Cavite, however, he was asked for his vaccination card to which he cannot provide yet because of the situation mentioned above.
I am hoping for an immediate response.

 Thank you and God bless!
