Good day! I would like to ask for the following information:

1. Number of reported cases of water-borne diseases such as cholera, typhoid fever, leptospirosis, hepatitis A and schistosomiasis in Laguna and Calamba City

2. Please also specify the town/barangay and year in table form as to when and where the disease was reported.

4. Please also include the annual morbidity and mortality rate of the said diseases.
