Good Day FOI,

I would like to ask request on DOH to update my vaccine record since my 1st, 2nd dozed and booster shot does not yet appear. I will be using the details to travel abroad. 

Here are my details:
Origin: manila

1st dose: Astrazenica 07/26/2021 city of manila
2nd dose: Astrazenica 09/20/21 city of manila
Booster shot: Astrazenica 02/05/22 city of manila

You asked for Vaccine cerrificate update.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
