Hi! I would like to request another vaccine card because mine got lost. I dont have any pictures of my vaccine card however I remember when and where I got vaccinated. I got my first dose at December 24, 2021 in Vincenzo sagun gymnasium Zamboanga del sur and my second dose at December 22, 2021 at the same place. I tried searching for my record in VaxCertPh, it says that I dont have any record. I hope my request gets fulfilled for I will be using it for requirements to my work

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
