Dear FOI PH,

I'm following up with DOH on their email regarding my vaxcert. I tried calling their hotline but no one is picking up. I would like to have my vaxcert updated in the system as my second dose is not reflected. I'll be needing the vaxcert to travel abroad as I'm not able to get an earlier appointment from BOQ. (Earliest possible is June 28 2022, my flight is at June 26, 2022) I sent emails and inquiries but no one is responding.

Below is my vaccination details:
1st dose (Astrazeneca) - Quezon City, NCR (05/14/2021)
2nd dose (Astrazeneca) - Quezon City, NCR (08/06/2021) = NO RECORD

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
