Dear Sir/Madam
I was vaccinated for covid19 last July 01, 2021 & July 29, 2021 held at Top Coastal, Parañaque, conducted by Manila Doctors Hospital, with booster shot last January 29, 2022 held at NAIA Terminal 4, Pasay City , conducted by Bureau of Quarantine.
As of today, i still haven't received the VaxCert online. I have been requesting it online but to know avail.
I am requesting for the immediate action on this as my company will send me abroad for a business trip. I hope you will give importance to my request.

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
