Lost of vaccine card

Requested from DOH by C. Gallofin at 10:42 PM on Jun 18, 2022.
Purpose: School Requirements
Date of Coverage: 06/05/2022 - 06/18/2022
Tracking no: #DOH-165605332985

Gallofin 10:42 PM, Jun 18, 2022

I would like to raise a concern regarding my vaccination record. I lost my vaccination card this week. I'm fully vaccinated but when I checked my record through https://vaxcert.doh.gov.ph/, it says that they have no record of my vaccination.

I was vaccinated in via UC SHS. BLDG. J. ALCANTARA ST. Cebu city. My first dose was on September 29, 2021 and the second dose was at October 27, 2021. As I have said above I lost my vaccination card and wasn't able to take photo of it, but I do have pic of proof. That they send me a message from Cebucityvax.

Sincerely yours,
Carmelle A. Gallofin
