Hi! I would like to request for another vacine card because mine got lost. I donvt have any pictures of my vaccine card but I remember when and where I got vaccinated. I got my first dose on June 7, 2021 and my second dose on July 5, 2021 at Pag-asa Elementary school under the name Lulette Leonardo De Guzman. I also have the I.D number which is CBC00457-0607-249.

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
