Hello! I would like to raise a concern regarding my vaccination record/card. I lost my vaccination card this June 2022. I am fully vaccinated but I don't have any photos of my vaccination card (or any other copy).

 I got my first dose of the vaccine in the Tacloban City Convention Center on November 9, 2021 and my second dose was on December 1, 2021 in the same location under the name Dovy Macabante. As I said I do not have a photo of the card itself, but I do have proof that I was vaccinated on those dates.

with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
