Hi! My name is Mark R. Corpin, I got vaccinated in Naval, Biliran. Today, I just realised I lost my vaccination card and I badly needed it. Is there a way I can reprint my vaccination card? Thanks!

Response to Request

Your FOI request is approved.

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.

Kindly coordinate with the LGU at Naval Biliran where you received your vaccination and request for assistance to provide a new vaccination card.
