I would like to raise a concern regarding my vaccination record. I lost my vaccination card last January. I'm fully vaccinated but when I checked my record through, it says that they have no record of my vaccination. 

I was vaccinated in the Tagum Public Market for the first does and Robinsons for the second dose. My first dose was on 11/30/21 and the second dose was at 01/17/22. I was able to take a photo of my vaccination card if needed. Thank you.

About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
