Hi! I would like to request another vaccine card because mine got lost.I got my first dose at November 11, 2021 in Barras Dako, Mabinay and my second dose at January 13, 2022 at the same place under the name Carl Anthony Macumao. I hope my request gets fulfilled for I will be using it to travel in Manila next month

Response to Request

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
