Hello!

I would like to request correction of my middle name in my vaccination record and certificate on vaxcertph. 
The website / vaxcert record for my name is set to COLIGO as my middle name but it is COLICO
Below are all the details.

Birthday: June 20,2000
Sex: FEMALE

2 ways to get your VaxCertPH after getting vaccinated:
a) Through website https://vaxcert.doh.gov.ph/
b) Go to LGU for assistance (Request more information at the Municipal/City Health Office)
FOR those whose vaccination records could still not be found yet, just check from time to time to get updates.
