Hi I would like to follow up on my request for a Vax Cert. I sent a request last January 19,2023 and have not received any feedback from you as of this writing. My reference ID is 4124813
I was vaccinated in San Fernando, La Union for 1st dose of Janssen last July 28,2021 and booster shot on January 2,2022 and September 27,2022 respectively. Details were sent along with the photo of the vaccination card.

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph. You may also review the FAQ section of the eFOI site: https://foi.gov.ph/help.
